
This course is for clinics who are interested in learning the key tenets of the Patient Centered Medical Home (PCMH) model. Interested clinics may be interested in eventually pursuing accreditation as a PCMH through the National Committee for Quality Assurance, or may intend to implement some of they main learnings and practices without gaining accreditation.
The PCMH Toolkit is designed to provide tools, resources and support to clinics and health centers with adopting key components of the patient-centered medical home (PCMH) model. While the concepts and topics covered throughout the courses in this Toolkit are grounded in the National Committee for Quality Assurance (NCQA) 2017 Standards for PCMH, completing this Toolkit alone will not fully prepare clinics to achieve recognition from NCQA. The goal behind this Toolkit is to support clinics in the implementation of key elements of the PCMH model that have had the biggest impact on clinics that completed their transformation to PCMH – impacts on the clinic, staff, patients and quality.
